Résumé

Introduction : Les conflits neuro-vasculaires (CNV) de l'angle ponto-cérébelleux se définissant comme une interaction entre un nerf et une structure vasculaire normale ou pathologique (artère, veine, malformation vasculaire artérielle ou veineuse), traduisant une compression et une souffrance du nerf dans cet espace. Ces conflits assez rares représentent une urgence neurosensorielle pouvant mettre en jeu le pronostic fonctionnel du patient. Le but de cette étude était de rapporter nos deux cas afin de rappeler les aspects cliniques, diagnostiques et thérapeutiques du CNV.

Observations : Il s’agit de deux patients, âgés de 45 ans et de 39 ans, sans antécédents particuliers, ayant présenté un vertige rotatoire sans autres signes associés. Face à la persistance de la symptomatologie, malgré une prise en charge médicale adéquate, les patients sont orientés au centre tertiaire. Les résultats de l'IRM (Imagerie par Résonance Magnétique) cérébrale révélaient un conflit vasculo-nerveux entre la VIII^ème paire crânienne et l’artère cérébelleuse antérieure inférieure(ACAI) de deux côtés chez le premier patient et unilatéral gauche chez le deuxième. Après concertation  pluridisciplinaire et suite au refus de la prise en charge chirurgicale, nous avons instauré un traitement médical d’attente en cas de crise à base d’acétylleucine 1000mg par jour. Comme évolution, nous avions noté une légère accalmie de la symptomatologie.

Discussion-conclusion ; Les conflits neuro-vasculaires existent mais rares. Il faut y penser devant des vertiges récurrents après un traitement médical bien conduit. L’IRM cérébrale permet de poser le diagnostic de certitude. Le traitement est constitué par un traitement médical et un traitement chirurgical par décompression neuro-vasculaire.

Two observations of recurrent dizziness revealing an acoustic-facial conflict.

Abstract

Introduction: Neurovascular conflicts (CNV) of the cerebellopontine angle are defined as an interaction between a nerve and a normal or pathological vascular structure (artery, vein, arterial or venous vascular malformation), reflecting compression and suffering of the nerve in this space. These relatively rare conflicts represent a neurosensory emergency that can jeopardize the patient's functional prognosis. The aim of this study was to report our two cases in order to recall the clinical, diagnostic and therapeutic aspects of CNV in two patients in Yaoundé, Cameroon.

Observations: These are two patients, aged 45 and 39 years, with no particular history, who presented with rotational vertigo without other associated signs. Faced with the persistence of symptoms, despite adequate medical care, patients are referred to us. The brain MRI (Magnetic Resonance Imaging) done in our two patients showed a vasculonervous conflict between the VIIIth cranial pair and the anterior inferior cerebellar artery (AICA) bilaterally in one and on the left in the other. Faced with the refusal of surgical treatment, a waiting medical treatment in the event of a crisis based on acetylleucine 1000 mg twice a day after multidisciplinary consultation is adopted in our patients with a slight lull in the symptoms. Discussion-conclusion; Neurovascular conflicts exist but are rare. This is something to think about when faced with recurring dizziness after well-conducted medical treatment. Brain MRI allows for a definitive diagnosis. The treatment consists of medical treatment and surgical treatment by neurovascular decompression.
