RESUME

Introduction : L’intubation trachéale rétrograde est une des techniques alternatives de la fibroscopieen cas d’intubation difficile prévue. Des modifications de cette technique ont été rapportées depuis quelques décennies. Nous décrivons la faisabilité de l'intubation rétrograde en utilisant un mandrin d’Eschmann.

Observation : Les auteurs rapportent un cas d’intubation trachéale rétrograde chez un homme de 42 ans, opéré d’une libération de bride rétractile de la région cervicale, séquellaire d’une cellulite cervico-faciale. L’intubation antérograde était impossible devant l’impossibilité d’extension cervicale.   Une trachéotomie était réalisée. Le mandrin béquillé d’Eschmann était glissé en direction céphalique pour être récupéré dans la cavité buccale afin de pouvoir guider la sonde d’intubation en intra-trachéale. 

Commentaires : Ce rapport de cas offre l'occasion de discuter de notre expérience et de décrire la faisabilité de l'intubation trachéale rétrograde en l’absence des dispositifs recommandés. Le mandrin d’Eschmann est une alternative adapté à notre contexte du fait de sadisponibilité et de son caractère malléable et flexible.

ABSTRACT

Introduction: Retrograde tracheal intubation is one of the alternative techniques of fibroscopy in the management of difficult intubation. Modifications to this technique have been reported in recent decades. We describe the feasibility of retrograde intubation using an Eschmann stylet.

Observation: The authors report a case of retrograde tracheal intubation in a 42-year-old man, operated for a retractile scar in the cervical region, complication of cervico-facial cellulitis. Anterograde intubation was impossible due to the impossibility of cervical extension. A tracheostomy was performed. Eschmann stylet crutched was slid in the cephalic direction to be retrieved in the oral cavity so that the intubation tube could be guided in the trachea.

Comment: This case report provides an opportunity to discuss our experience and describe the feasibility of retrograde tracheal intubation in the absence of the recommended devices. The Eschmann stylet is an alternative adapted to our context because of its availability, hismalleability and flexibility.
