RESUME

La cornée est le premier dioptre du système optique de l’œil. Une opacification cornéenne centrale entraine une baisse de la vision. Nous rapportons un cas d’opacification cornéenne centrale pénalisant la vision et qui s’est amélioré après utilisation de prisme. Il s’agit d’un jeune homme de 19 ans avec une opacification cornéenne centrale de l’œil droit, ayant résulté d’une projection d’un objet métallique 3 ans auparavant. L’acuité visuelle était réduite à la perception de mouvement des doigts. L’introduction du prisme de 1 dioptrie à base nasale dans la correction optique augmentait l’acuité visuelle de l’œil à 10/10. Un prisme de 1 dioptrie à base temporale était incorporé dans la correction de l’œil controlatéral pour éviter la survenue d’un strabisme. Le suivi du patient n’a rapporté aucune anomalie décelable.

L’utilisation de prisme est une alternative à la kératoplastie et s’avère efficace pour la prise en charge de l’opacification cornéenne, surtout dans les pays à faible revenu où la kératoplastie n’est pas à la portée de tous.

ABSTRACT

The cornea is the first interface of the optical system of the eye. Central corneal opacification reduces visual acuity. We report a case of central corneal opacificationpenalizing vision and which improved after using prism. He was a young man of 19 years with central right corneal opacification as a result of a projectionof a metallic object 3 years before. Visual acuity was reduced to the perception of movement of the fingers. The introduction of the prism of 1 diopter at nasal base in the optical correction increased visual acuity of the right eye to 10/10^th. A temporal-based 1 diopter prism is incorporated into the correction of the controlateral eye to avoid the occurrence of strabismus. Patient follow-up reported no detectable abnormalities.

The use of prism is an alternative to keratoplasty and is effective for treating corneal opacification, especially in low-income countries where keratoplasty is not within everyone’s reach.
